Position Summary Coordinate all patient care functions for Oncology ambulatory office(s) that includes: referrals, authorizations, scheduling infusion, and coordinating associated imaging, labs and specialty visits
Essential Job Outcomes
Schedule patients for in-office care according to the Oncology office workflow. Utilize appropriate follow through in documenting messages.
Coordinate scheduling of pre-infusion and post-infusion care between office and MHS hospital. Maintain accurate schedules and update all locations to any changes.
Actively work with patients, physicians, and MHS hospitals to facilitate patient’s need regarding oncology treatment. Includes assisting with obtaining durable medical equipment for patient as required.
Obtain referral authorization for patient(s) being treated within MHS hospitals and ambulatory offices.
Obtain infusion order sets, authorizations and required diagnostics.
Obtain medical records from outside facilities and arrange courier service when needed to assure the necessary information is available to the surgeon prior to the patient’s visit. Verify patients’ current demographics, insurance, and pharmacy information. Check-in all patient appointments once they have arrived for care.
Review pre-visit instructions with patient in the office and/or over the phone to ensure he is properly prepared for the visit. Communicate effectively with patients regarding changes in schedule, issues, and physician office concerns.
Escort patient to vehicle, parking lot if needed, and/or around hospital for testing to assure satisfaction and safety.
Build/maintain patient tracking, care reports, and patient forms using Excel, Word Documents and Outlook.
Manage patient FMLA and Disability forms.
Locate provider to coordinate care in case patient emergency or urgent hospital need.
Perform any additional or miscellaneous duties as requested by the management team within the scope of knowledge and ability.
