General Duties:The LPN is the technical expert for performing, reporting, and recording assigned nursing care, including, but not limited to: Administers oxygen therapy Medication administration Completes clinical reminders with appropriate consults and alerts as indicated Ensures medications/supplies are not expired and removed from inventory Accurately performs delegated procedures and treatments Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. EKG, lab draws Data collection and documentation intervention of patient visit in applicable nursing note. Based on the patient's goals, reinforces through health coaching and other methods, patient and family health education, to include self-management, prevention and wellness. Assists in medication reconciliation. Notifies the provider of changes Measures, records vital signs and enters them in CPRS. Applies skin tests and measures and documents results. Visual acuity tests Performs Monofilament exams Closes encounters daily Checks the Crash Cart and AED per facility policy Complete the Blood Glucose meter check daily, as applicable. Vaccine Team/ Primary Care Team Float: The Licensed Practical/Vocational Nurse (LPN) is organizationally aligned under Ambulatory Care Services, where he/she will work as part of and in collaboration with a larger clinical team to provide primary care and vaccine delivery services to the VA Ann Arbor Healthcare System Veteran patient population. The LPN will be assigned to one or more patient aligned care teams (PACT) within the healthcare system and will assume routine clinical care delivery and administrative roles when not performing vaccine related duties. The LPN is directly responsible to the PACT Vaccine Support & Float Team Nurse Manager. Sub-Specialty Team Float: The Licensed Practical/Vocational Nurse (LPN) is organizationally aligned under Ambulatory Care Services, where he/she will work as part of and in collaboration with a larger clinical team to provide sub-specialty care services to the VA Ann Arbor Healthcare System Veteran patient population. The LPN will be assigned to one or more sub-specialty clinics within the healthcare system and will assume routine clinical care delivery and administrative roles consistent with his/her sub-specialty clinic assignment. Some of the areas/clinics the LPN could be assigned to, but are not limited to, are Hematology/Oncology (Hem/Onc), Integrative Pain, PM&R (Pain Management & Rehabilitation), Dermatology, Women's Health, Infectious Disease, Pulmonary, Neurology, Arthritis, Gastrointestinal (G.I.), and Renal. The LPN is directly responsible to the Sub-Specialty Care Clinics Nurse Manager.
