The Hunter Holmes McGuire VA Medical Center, Richmond, Virginia is hiring for a Registered Nurse (RN), Clinical Inpatient Transfer/Flow Coordinator who will be directly responsible to the Chief of Staff. The Registered Nurse will collaborate with the interdisciplinary health care team, community agencies/facilities, and patient/families/significant others to ensure continuity of care across all levels of care ensuring timely placement of patients into the appropriate level of care. The Registered Nurse, Clinical Inpatient Transfer/Flow Coordinator is responsible and accountable for: Coordinating transfer-in of Veterans from community/non-VAMC hospitals. Ensuring accurate entries to the Daily Fee Report and Electronic Morning Report document, updating entries from consultations and reports from VAMC and community clinicians. Providing direct patient care for veterans assigned to the Chronic Disease Management clinic for Ambulatory Care Sensitive Conditions (ACSC). Compiling reports of data from discharges, refusals/denials. Attending and participating in Bed Huddles, Transitioning Level of Care meetings, phone conferences, collaborative meetings, phone conferences, etc. Other responsibilities, as assigned by the supervisor and required by VISN 6 directives. Work Schedule: Monday - Friday 7:30 - 1600 (Rotating Shifts as Medical Center Needs Dictate) Friday/ Saturday/Sunday, 7pm - 7:30am, Thursday 12am - 8:30am (Rotating Shifts as Medical Center Needs Dictate) Telework: Not Available Virtual: This is not a virtual position.
