This position is located in Nursing Service, and is part of a Patient Alignment Care Team (PACT). This position is accountable to the Nurse Manager in ambulatory care/Primary Care/PACT and works under the direction of the Registered Nurse (RN). Responsible for the treatment, observation and care of the Veteran within the scope of LPN functions. Assist in the overall PACT clinic management, functions and operations. Facilitate patient flow, thorough check in and checkout process. Provide patient education and/or instructions. Work with care team and community resources. Participate in performance improvement activities and other activities that enhance patient care and clinic operations. Maintain patient's privacy and confidentiality. Document pertinent information in the electronic medical record. Assist with coordination of all services for the patient with PACT Team and group visits. Work with the patient, significant other and health care team to establish patient centered short- and long-term goals. Demonstrate knowledge and skills sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to the Registered Nurse.
