Dignity Health has an exciting leadership Transitionalist Medical Director opportunity for either an Internal Medicine or Family Medicine Physician located at our Phoenix, Arizona facility.
The Transitionalist Medical Director will engage in direct patient care activities and medical/clinical practice as the principal component of his/her overall professional responsibility.
The Transitional Care Medical Director Reports directly to the DHMG CMO. Patient care & Director Activities will include, but are not be limited to: 1. Assessment, evaluation, diagnostic determination, on-going treatment and clinical management, necessary referral for consultation, care planning and modification, medications reconciliation, risk assessment, and, coordination of care. 2. Direct contact and consultation with patient family members, advocates, and others to ensure effective communication of patient status and care plans, and to encourage/support an open visitor- friendly environment within the Transitional Clinic. 3. Extensive and on-going communications with community principal care physicians who routinely manage the care of patients. 4. Day-to-day patient care management activity that includes professional input from all members of the patient care team. 5. Accurate and timely completion of all clinical and professional records and documentation to assure coordination of care. 6. Provides one on one and group consultation to all providers as needed regarding clinic policies and processes, patient care and communication challenges, as well as innovative care ideas. 7. Oversight of the Nurse practitioner 8. Leads regularly scheduled Operations Meetings with office manager and other key staff members. 9. Monitors provider performance on clinical quality, meaningful use, population health, and productivity on a quarterly basis and ensures that all providers receive reports and communication on a timely basis. 10. Perform other related duties incidental to the work described herein.
Goals of the Transitional Care Medical Director: 1. Increase the number of referrals of high risk patients from the hospital/ Ed to the TCC. 2. Decrease the readmission of high risk patients. 3. Decrease admission of Ambulatory Sensitive Conditions to the acute care setting. 4. Patient Satisfaction
