We are seeking a dedicated and compassionate Staff Geriatrician to provide both primary and specialty care to older veterans, particularly those with multiple chronic conditions such as dementia, across ambulatory care and long-term care facilities. The successful candidate will manage a panel of geriatric primary care patients, providing care through face-to-face visits, telehealth consultations, and e-consults. Expertise in treating common geriatric conditions, including hypertension, diabetes, coronary artery disease, chronic renal insufficiency, osteoarthritis, and complex geriatric syndromes such as dementia, gait instability, urinary incontinence, and polypharmacy is required.
This role offers the opportunity to collaborate daily with an interdisciplinary team (IDT), including physicians, nurse practitioners, and physician assistants, ensuring comprehensive and coordinated care. The Geriatrician will provide leadership and guidance to clinical staff, including supervising nurse navigators, nurse practitioners, and physician assistants. Additionally, the incumbent will serve as a mentor to medical trainees, including fellows, residents, and medical students, providing hands-on teaching, and supporting their professional development.
The ideal candidate will have strong communication and documentation skills, ensuring that all medical records and patient encounters are completed accurately and in a timely manner. Effective communication with patients, families, treatment teams, and external partners is essential. The role also includes participation in quality improvement activities and applying new knowledge to enhance patient care across the VA system. Occasional involvement in long-term care facilities may be required to ensure continuity of care for veterans.
